Who is eligible for assistance?

	A child between the ages of 2-17, who has been diagnosed with a life-threatening medical condition
	The child must be in treatment at the time of the referral
	The child is a resident of Chino Valley

Who can refer a child for assistance?
Children may be referred to receive assistance by any of the following sources:

	Licensed medical professionals involved in the treatment of the child
	A parent or legal guardian
	A pastor/minister of the child and family
	The principal of the child's school

How does the approval process work?
The board of directors reviews all applications monthly. Notifications will be sent upon physician verification.
